Enrollment Forms

Once accepted into a preschool classroom we will need the following items:

For Every Child:
• $20.00 Registration Fee
• A copy of the child's CERTIFIED birth certificate (hospital copies are not accepted)
• A copy of current immunizations (see link below for schedule)
• Background Form
• Child Release form
• Dental Screening form
• Financial Form
• Medical Exam Form
• Parent Permission Form
• Volunteer Form
